Aim

To provide the competence, including the practical skills, required by anyone who is involved in accident or incident investigation at work

Objectives

By the end of this course you will:

• Understand what accidents and incidents are (including near misses) and how they are caused
• Understand the reasons for reporting and investigating accidents and incidents
• Be able to carry out an accident investigation at work and recommend improvements to prevent it happening again
• Understand the main legal requirements, including the Reporting of Injuries, Diseases and Dangerous Occurrences Regulations 1995 (RIDDOR).

Description

This course ensures participants understand how to investigate accidents and incidents effectively at work. It is based on a number of real accidents and includes a practical exercise to allow the participants to practice the investigation skills taught on the course. It also includes the legal requirements for reporting injuries, diseases and dangerous occurrences.

Who should attend?
Anyone who is or may be involved in accident or near miss (incident) reporting and investigation at work, including Facilities Managers, line managers and health and safety staff.

• Accidents and incidents
Definitions and possible outcomes, costs of failures
• Accident reporting
Internal and external reporting requirements, including what needs to be reported under RIDDOR and how
• Accident causation
How accidents happen – immediate and underlying causes
• Accident investigation
Purposes of accident investigation
• Deciding how thorough accident investigations should be
• Investigation techniques, including evidence gathering (observation, examining, documents and interviewing) and report writing
• Practical accident investigation exercise, based on real accident scenario
• Identification of actions required to prevent recurrence.
